Terms and Conditions

The Proposed Work is to be paid in full at the time service is rendered by cash, check, or credit card unless other arrangements have been made in advance.

Returned checks are subject to a $30.00 service fee in addition to the amount of the invoice plus any collection fees if they become necessary including a 1 1/2% per month (18% annual) interest rate on unpaid balances after ten (10) days from date of work order / invoice.

All materials and parts furnished in the Proposed Work remain the property of My Heating and Cooling until payment in full has been received and processed for settlement through proper channels with the transfer of funds in full payment of the Proposed Work as agreed upon. If settlement has not been completed within thirty (30) calendar days from date of the work order / invoice, My Heating and Cooling retains the right to remove the materials and parts at the expense of the property owner where the Proposed Work was completed.

Statement Of Acceptance is a part of all work order / invoices

I, _____________________________________________________ as the owner of the property located at ____________________________________________ in the city of ___________________________ in the State of Illinois have read, understand and hereby authorize the Proposed Work and agree to the Terms and Conditions as stated on both sides of this form. I warrant that I am the owner of the property where the Proposed Work is to take place and grant My Heating and Cooling Company and their agents access to complete the Proposed Work. I further understand that cutting or removal of finished surfaces may be required to complete the Proposed Work and the cost to repair, replace or refinish these surfaces is NOT included in the amounts for the Proposed Work. I understand and agree that the existing conduit, wiring, duct work, or equipment with associated apparatus may no longer be serviceable or repairable and I agree to hold My Heating and Cooling Company, its owners, agents and employees blameless to any damages or destruction to these items in the course of completing the Proposed Work. I acknowledge and accept the schedule of fees and or interest charges for returned checks or credit card transactions used to pay for the Proposed Work including all costs associated with the collection of these fees. A deposit in the amount of $____________________ has been provided towards the completion of the Proposed Work.

All worked performed per My Heating and Cooling Company's "Terms and Conditions" in designated service areas of the Metro East Area of Illinois. My Heating and Cooling Company is a division of Quick Fix Home Repair, Inc. and retains the right to accept or reject any work order or request for service received from this site based on availability of man power, materials and scope of work or as determined best for the owners and or their representatives.
